Define which repair the closeout record describes

A mining structure repair may involve a small local detail within a much larger asset: a conveyor support, transfer structure, chute frame, platform or another identified assembly. The NDT closeout record should describe the actual authorized repair area rather than implying that the entire structure has been assessed. Start with the owner's asset identity, the repair work order and the engineering or maintenance reference that defines the scope. If site personnel use an informal name for the location, retain it as a helpful alias while connecting it to the controlled record.

The reporting objective is to connect the repaired zone to the examinations actually performed and the disposition references needed by the responsible team. It is not to determine structural capacity, prescribe a repair method or authorize continued operation. Those decisions require the appropriate engineering and site processes. A good closeout package makes their evidence easier to retrieve: which detail was addressed, which configuration was examined, what remains unresolved and where the authorized decision can be found. This guide explains an editable record structure that supports that task without inventing technical acceptance criteria.

Anchor the repair zone to a reproducible physical reference

Use the approved drawing, marked location plan or other controlled reference to identify the repair zone. Record the drawing number and revision, the relevant member or detail identifier and the orientation convention needed to interpret it. A photograph can help a recipient recognize the area, but a close-up of a weld may be insufficient to relocate it within a large transfer structure. Keep an overview relationship and a detail relationship where the project records provide them. Describe any temporary identification used during the work and how it maps to the owner's asset record.

Avoid making a filename or a verbal description the only location reference. Terms such as near the drive end can become ambiguous when several similar assemblies exist or when different teams approach the equipment from different directions. If the repair location was clarified after the initial request, preserve the revised reference and the source of the clarification. An unresolved location discrepancy belongs in the closeout record as an open question. Choosing a plausible member identifier simply to complete the table can connect a valid examination to the wrong physical detail.

Preserve the authorized repair configuration and its changes

The examination needs to be related to the configuration that existed when it was performed. Retain the applicable repair drawing, instruction or disposition reference and identify changes issued during execution. If a revised detail replaces an earlier proposal, state which revision governs the completed work and how the existing examination records relate to it. Do not assume that a report issued earlier automatically covers a later configuration. The responsible technical party must determine the consequence of the change, and the record should make that review possible.

NASA's configuration-management guidance explains the general importance of keeping product information consistent with controlled changes. It is cited here as a documentation principle, not as a mining requirement. The editorial application is to preserve the link between the repair's authorized description and the physical state examined. NDT Connect does not synchronize engineering drawings or approve configuration changes. The report author enters the verified references and records the review outcome supplied by the responsible organization.

Separate the original finding from the repair examination

Keep the original finding identifiable even when the repair has been completed. Its report, location reference and disposition may explain why the work occurred. The later examination should describe the condition and zone it actually covers, with its own date, method, procedure and result. Combining both events into one undated statement can erase the sequence that the client needs to understand. A clear stage field can distinguish the initial observation, an intermediate examination and the documented examination of the completed repair. When several events occur during one shift, retain the event reference and recorded timing needed to distinguish their sequence; the shared calendar date alone may not explain it.

An earlier finding should not be overwritten with a later satisfactory result. Preserve the relationship between them through references so the recipient can see what was changed and what evidence supports the later state. If the repair scope expanded, identify the additional area and the corresponding examination rather than stretching the original report's description after the fact. The closeout index can summarize the sequence, while the underlying reports retain their detailed findings. This approach also helps when different qualified personnel performed the original and subsequent examinations under different authorized work stages.

Record coverage limits in terms the action owner can use

A closeout record should distinguish the intended examination extent from the extent actually documented. Identify the examined zones using the approved reference and record any reported access, geometry or other relevant limitation. Do not translate partial coverage into a statement that the whole repaired member was examined. If the examiner supplied a bounded conclusion, preserve the boundary in the result table and the summary. A recipient should not have to search a remote appendix to discover that the conclusion applies only to one accessible face or a defined part of the detail.

Keep different kinds of incompleteness separate. An area outside the authorized repair scope is not the same as an area requested but inaccessible; an examination completed with its report missing is a different problem again. Assign a clear action to the appropriate owner: scope confirmation, technical review, additional authorized work or document retrieval. The template should not automatically select the next inspection method or declare that an inaccessible area is acceptable. It should preserve the facts so the responsible party can make and document the required decision.

Keep temporary measures and operating decisions visible

A repair package may refer to temporary measures, restrictions or a staged restoration plan established by the responsible engineering or site authority. If those references exist, record them accurately and identify the document that controls their current status. Do not invent a loading restriction or remove one because a local NDT report has been issued. The examination result is evidence within the wider process; it is not by itself an instruction to change how the structure is used. Make that relationship clear in the dossier's summary and outstanding-action record.

When an authorized restriction is later revised or closed, retain the decision reference rather than deleting the earlier context from historical reports. The closeout index can show the current controlling document while preserving the sequence of decisions. Avoid an unqualified ready for service checkbox in an NDT worksheet unless the organization's approved process defines who can complete it and what evidence it represents. A more useful reporting field identifies the release or operating-decision reference and leaves it unresolved when that document has not been supplied.

Connect technical results to their source records

For each examination, retain the report number and revision, method, procedure reference, examined zone and date. Link equipment and personnel evidence according to the actual project requirements, without assuming that a current account record proves historical suitability or authorization. Where the result depends on a marked sketch, source image or instrument file, identify the supporting record in the approved storage arrangement. The report should distinguish a source file from a presentation image or a later explanatory annotation. File references need to remain intelligible after the package leaves the author's workstation.

NIST's research on connecting design, manufacturing and inspection information considers the difficulty of linking lifecycle evidence. For this repair worksheet, the practical link is deliberately simple: one defined repair zone, one work stage and the documents supporting the reported examination. The spreadsheet and PDF are not an automated engineering model. An accountable author still checks that the linked records describe the same asset, configuration and event rather than relying on a common project name to establish the relationship.

Use a fictional conveyor-support repair to test the package

Imagine a fictional conveyor support identified as CV-08, with a repair zone Z-3 defined on drawing R-18 revision C. The initial finding is documented in report EX-101. The approved repair changes one local detail, and a later examination is recorded in EX-118. A second nearby area, Z-4, was discussed during the work but has not been added to the authorized repair scope. The closeout index should identify Z-3 and its evidence while keeping the status of Z-4 explicit. It should not silently include the neighboring area in a completed-repair statement.

Suppose the final examination record for Z-3 also notes a defined coverage limitation that requires review. The coordinator enters the report reference, the limitation and the engineering-review action separately. The package can be complete as a delivery of the available records while the technical action remains open. If the responsible authority later issues a disposition, the index references that decision and its scope. The example illustrates evidence organization only; it does not determine whether the fictional structure is acceptable, what further examination is needed or whether operations may resume.

Build a closeout table that separates evidence and decisions

Use columns for asset and repair-zone identity, controlling drawing, work stage, examination reference, documented coverage, reported result, open action and responsible decision reference. Do not make the status column carry all of those meanings. Examination performed, report received, technical review completed and release authorized are different events. A separate document-status field helps the coordinator chase missing records without changing the examiner's technical result. The downloadable worksheet uses this separation so unresolved matters remain visible even when other parts of the package are ready.

A useful row should be readable on its own but not duplicate every underlying report paragraph. Keep the zone and stage clear, include the exact report revision and use concise references for longer explanations. Where several examination methods address different aspects of one repair, use separate rows or an explicitly organized subtable. Avoid a generic methods completed entry that obscures which method covered which area. Before importing, replace illustrative values with verified project data and explain any blank field whose meaning would otherwise be uncertain to the recipient.

Prepare the PDF and its companion records deliberately

The free digital reporting workspace can import editable tables from a Word or Excel template. Preview the imported content, choose the relevant header row and adjust the result table to match the actual repair record. Enter the controlling requirements, equipment references, personnel and observations in the appropriate report fields. Keep a detailed drawing or instrument file in the approved evidence system and enter its reference; the editor is not a structural-analysis tool or a native NDT data viewer. Review the generated PDF for readable zone identifiers and complete scope notes.

Add company and client logos only where their use is authorized. A logo or an issued status does not establish an engineering release. The report's conclusion should express the responsible author's actual finding within the stated examination scope. A guest can download a draft PDF; a verified free account can save a private report with a tracked number and revisions. Use that numbering alongside the owner's repair and asset identifiers, not in place of them. The receiving team needs both a stable report reference and a reliable way to connect it to the structure.

Reconcile the open-action list before calling the package final

Review each outstanding action with its owner and the evidence actually supplied. A missing report should close only when the intended document and revision are available. A technical limitation should point to the responsible review or disposition, not merely to an email saying the package was received. If a drawing revision changes after the examination, preserve the review of its effect on the existing evidence. The table should explain the current state without erasing the fact that an earlier package was provisional or incomplete in a defined way.

Where a final package includes unresolved matters by explicit agreement, describe the agreement and its limits. Do not rename an open technical issue as an administrative note to make the summary look complete. If a previously delivered report is corrected, identify the replacement revision and follow the organization's distribution process. The closeout coordinator should be able to show which evidence supports each closed item and who owns each remaining one. This makes the dossier useful during maintenance planning and later record review without overstating what the inspection team has decided.

Perform a retrieval review from the recipient's viewpoint

Select one repaired zone and trace it through the delivered package: physical reference, authorized repair revision, examination stage, result and decision reference. Then choose an unresolved or restricted item and check whether its owner and next action are clear. Use the actual delivery folder or channel, not only the author's working copy. If the references require knowledge that exists only in a team member's memory, add the missing explanation before handover. A small retrieval exercise can expose an incorrect drawing revision or an attachment that was omitted during packaging.

The procedure draft workspace can support preparation of a document for responsible technical review, and the inspection quote comparison can help compare actual proposed scope when additional work is requested. Neither decides the repair's technical adequacy. The closeout record succeeds when it preserves the distinction between a reported examination, a controlled engineering decision and an operational authorization, while making the connections between those records easy for the next person to follow.

Questions about this workflow

Does a completed NDT report authorize the structure to return to service?

No. Keep the examination finding separate from engineering and operational decisions, and reference the actual responsible authorization.

Can the closeout index cover only one repaired zone?

Yes. Identify the bounded scope clearly and avoid implying that unexamined or unrelated parts of the structure were assessed.

What if the repair drawing changes after examination?

Record the change and the responsible review of its effect on the existing evidence. Do not assume an earlier report automatically covers the later configuration.

Can an unresolved limitation remain in a delivered package?

Describe the limitation, owner and next action accurately. Any agreement to deliver a bounded package should remain distinct from technical acceptance.
